(3) Fees, Health Insurance, and other Information
How My Practice Is Funded
My services are not supported by any governmental, ecclesiastical, or non-profit agency.
I am on a "fee for service" basis only.
I seek to serve my clients in the most professional, efficient, and confidential manner possible.
Basically, I work for my clients by the hour as an individual professional and, in doing so,
I maximize quality, confidentiality, and personal contact, but minimize cost, overhead, and service time.
All services are scheduled by appointment only.
PLEASE TEXT ME WITH YOUR MOST CONVENIENT
OR DESIRED TIME AND DATE FOR AN APPOINTMENT
PLEASE REMEMBER THAT WE WILL BE HAVING OUR MEETINGS THROUGH EITHER:
(a) VIRTUAL TELEHEALTH INTERNET CONTACT (doxy.me/jerusalem)
(b) FACETIME THROUGH AUDIO/VISUAL CELL PHONE
(c) AUDIO ONLY THROUGH CELL PHONE or
(d) 'CHAT' ie MUTUAL real time CONTACT THROUGH MESSAGING
FEES
I offer two ways to pay for sessions
(*)My Primary method of receiving fees
for my for services is negotiated on the
first session based on a sliding scale
structure, and on ability to pay, calculated on the
annual gross family income
as reported on the IRS 1040
with a minimum of 60.00$
and a maximum of 100$ per session.
(*)Usually the fee is Payable through
Credit Card, which I have found is the
most efficient and preferred method of payment.
(*) Also if you find a need to do so, you
may opt to pay by Cash or Check by
mail to my office at
17811 Kyle Seale Pkwy, Box 38
SAN ANTONIO, TEXAS 78255
If you request it, once I have received
payment, I am willing to send you a receipt
which you can send to your insurance company
to either help satisfy your copay or deductible,
or be reimbursed by them to you.
(*) The second way to deal with fees is
through an "Employee Assistance Program"
funded by your employer.
There is no cost to you for these EAP sessions
and my fees for service are satisfied by your
employer directly to me.
You would need to contact the Human Resources
Office at your work to ask about this benefit.
The EAP is a short term, solution-oriented
psychotherapy. Most employers limit the number
of visits.
(*)I do charge 25$ fee for a non-emergency
"No Show"or a "Late Cancel"; but of course
there is no charge for emergencies or
for the unexpected demands of your work site.
.
(*)Before the first session I ask
new clients to text me pictures of their
(1) Texas Drivers License and
(2) Health Insurance card (front and back)
(3) The email address and preferred
phone numbers.
(*) As a safety measure, I prefer not to have
pictures of credit cards conveyed by texts or
computers.
(My practice is to write down the Credit Card
numbers in real time as you give the information
to me over the phone.)
I keep those numbers and all of your other
records and information, both on paper
and electronic, strictly confidential and
stored securely securely locked as is required
by state law.
Your file and the information in
it may only be released
by your hand written signature only.
